Below we inform you about the purpose for which the dental practice Züri Zahni collects, stores, or transfers your personal data. Additionally, we inform you of your rights regarding data protection.
Responsibilities
The responsible entity for processing your personal data, especially your health data, is the dental practice. If you have any questions regarding data protection or wish to exercise your data protection rights, please contact the practice staff or your dentist directly.
Collection and purpose of data processing
The processing (collection, storage, use, and retention) of your data is based on the treatment contract and legal requirements for fulfilling the treatment purpose and associated obligations. Data is collected on the one hand by the treating dentist during your treatment. On the other hand, we also receive data from other doctors, dentists, and healthcare professionals with whom you have been or are being treated, provided you have given your consent. Only data relevant to your medical treatment is processed in your medical record. The medical record includes personal information provided in the patient form such as personal details, contact data, and insurance information, as well as, for example, the informed consent discussion, collected health data such as medical histories, diagnoses, proposed therapies, and findings.
Retention period
Your medical record is stored for 10 years after your last treatment.
Disclosure of Data
We only transfer your personal data, particularly medical data, to external third parties if legally permitted or required, or if you have given your consent during your treatment.
Data may be transmitted to your health insurance provider or accident/disability insurance for billing purposes. The type of data transmitted is determined by legal requirements.
In individual cases, depending on your treatment and consent, data may be transferred to other authorized recipients (e.g., laboratories, other physicians, or dentists).
Revocation of Your Consent
If you have given explicit consent for data processing, you may revoke this consent in whole or in part at any time. The revocation or request to change a given consent must be made in writing. Once we have received your written revocation and the processing cannot be based on any other legal basis, data processing will be discontinued. The legality of data processing carried out before the revocation remains unaffected.
Access, Review, and Release
You have the right to access your personal data at any time. You may review your medical record or request a copy, provided your invoice has been paid. Providing a copy may incur a fee. Any costs, which depend on the effort required to prepare the copy, will be communicated to you in advance.
Right to Data Portability
You have the right to receive data that we process automatically or digitally in a common, machine-readable format—either for yourself or for a third party. This includes, in particular, the transmission of medical data to a healthcare provider of your choice. If you request a direct transfer of the data to another responsible party, this will only be done if technically feasible.
Correction of Your Information
If you find or believe that your data is incorrect or incomplete, you may request a correction. If neither the correctness nor incompleteness of your data can be determined, you have the right to request that a note of dispute be added.
